APPOINTMENT POLICY FOR SCHEDULING AND CANCELLATION/RESCHEDULING

  • Initial consults are 60 mins and default follow ups are 45 mins. An option for 30 min follows may be available upon request and at provider discretion.

  • To maintain placement on the roster, the Client must attend an appointment at least once during a 60 calendar day timeframe unless otherwise arranged with provider. Ongoing appointments (ex: every 2 weeks on Thursday and 2pm) are available as scheduling allows.

  • If the Client is late to the scheduled appointment, the original time will be observed and some session time may be lost.

  • Cancellation Policy requires the Client to give a minimum of 48-hours notice to cancel and reschedule a confirmed appointment so that the practitioner does not lose potential business. The Client’s insurance company cannot be charged for missed appointments. This $60 administration fee will be charged under the following circumstances:

    • If the cancel and reschedule request occurs in less than 48 hours.

    • If the Client is a no call/no show or more than 20 minutes late to their scheduled appointment time

    • The credit card (HSA and FSA are also permitted) on file will be charged for the administrative fee.

    • If there is no valid credit card on file or the card on file is declined, the Client will be issued an invoice that must be paid within 30 calendar days. If the invoice is not paid after 30 calendar days, the Client will receive an order from Small Claims Court for the invoice amount plus applicable court fees.

PAYMENT FOR SERVICES: INSURANCE

  • The Client must provide necessary insurance information to INSIGHT NUTRITION prior to the scheduled appointment. INSIGHT NUTRITION will contact your insurance company to verify coverage. If there are restrictions to your coverage, INSIGHT NUTRITION will be in touch to discuss options.

  • In the event a referral from a PCP is needed, it is the responsibility of the Client to submit the initial request to the PCP.

  • The Client agrees to contact INSIGHT NUTRITION if there is a change in insurance coverage and provide necessary documentation prior to the next appointment.

  • The Client gives INSIGHT NUTRITION permission to bill the insurance carrier for services.

  • The credit card (HSA and FSA are also permitted) on file will be charged for all copayment, coinsurance, or deductible amounts as determined by the Client’s insurance policy.

  • If there is no valid credit card on file or the card on file is declined, the Client will be issued an invoice that must be paid within 30 calendar days. If the invoice is not paid after 30 calendar days, the Client will receive an order from Small Claims Court for the invoice amount plus applicable court fees.

PAYMENT FOR SERVICES: ALTERNATIVES TO INSURANCE

If the Client is unable to use their insurance to cover nutrition visits because the plan includes limited or no nutrition counseling, or the provider is not in-network with the insurance carrier, the client understands there are alternatives for payment.

  • Self-pay option: the Client pays for individual visits out of pocket with the card on file. The Client will receive a Good Faith Estimate agreement with disclosed rates that are locked in for a 12-month period after which rates are subject to change.

  • The credit card (HSA and FSA are also permitted) on file will be charged for services before the start of each session.

  • Superbill option: the Client pays for services directly with the provider but can request a superbill (medical receipt). The Client will submit claims for reimbursement on their own if they have out-of-network insurance coverage.

  • If there is no valid credit card on file or the card on file is declined, the Client will be issued an invoice that must be paid within 30 calendar days. If the invoice is not paid after 30 calendar days, the Client will receive an order from Small Claims Court for the invoice amount plus applicable court fees.

PAUSE, ABSENCE, AND TERMINATION OF SERVICES

It is important to Rey Vanderpool and INSIGHT NUTRITION that clients respect their need for work-life balance. This includes honoring their time and understanding this work as relational, not solely transactional or at-will. Your participation and cooperation with the following policy is appreciated

  • INSIGHT NUTRITION and the Client equally reserve the right to terminate the provider/client relationship at any time if the work naturally comes to a conclusion or if the relationship is deemed not a good fit. This termination must be clearly communicated to the other party in writing or verbally.

  • Active clients are secured placement on the caseload roster. To maintain placement, the Client must attend an appointment at least once during a 60 calendar day timeframe unless otherwise arranged with provider.

  • If the Client requires a purposeful break from scheduling services, a conversation is required within 60 days of the last appointment to discuss options that will work for both parties.

  • If a client is absent for more than 60 calendar days from the last appointment without any attempt of communication to discuss maintaining placement on the roster, the Client will be terminated from services. If the Client desires to resume services after termination, they must communicate with INSIGHT NUTRITION to determine availability.

ELECTRONIC COMMUNICATION

INSIGHT NUTRITION cannot ensure the confidentiality of communication through email or text messages to discuss therapeutic content. Please utilize the secure message function on the client portal to communicate any information related to your treatment. Allow 1-3 business days for a response. Email, text, and phone calls are preferred for canceling or rescheduling appointments, only.
